What do I need to bring?

You will need to bring your lunch, a pen, your identification, and your USI with you to any of our training courses.
Please visit usi.gov.au and go to create USI  and follow the steps to create your USI number.

I have read and understood the course information and agree to abide by all the training and assessment requirements. I give permission for First Aid & Safety Training Pty Ltd to supply H & A Training Pty Ltd (90871) or Workcover NSW with personal details as above, including assessment results, for the purpose of issuing a Nationally Recognised Statement of Attainment. Should my Employer be paying for my attendance at any course, then my Statement of Attainment will be sent directly to them.
